Confidential Smartphone & Screen Compulsion Assessment

This 15-question screening assessment evaluates your relationship with your smartphone, digital boundaries, and whether screen time is impacting your sleep, focus, or relationships. Please answer each question as honestly as possible.

1. Do you find yourself spending more time on your smartphone than you realize?

2. Do you find yourself mindlessly passing time on a regular basis by staring at your smartphone?

3. Do you seem to lose track of time when using your smartphone?

4. Do you find yourself spending more time texting, messaging, or emailing as opposed to talking to people in person?

5. Has the amount of time you spend on your smartphone been steadily increasing?

6. Do you wish you could be a little less involved or preoccupied with your phone?

7. Do you sleep with your smartphone turned on under your pillow or right next to your bed on a regular basis?

8. Do you find yourself viewing and answering notifications at all hours of the day and night, even when it interrupts what you are doing?

9. Do you text, email, check social feeds, or browse while driving or doing other activities that require focused attention?

10. Do you feel your smartphone use decreases your productivity or focus at times?

11. Do you feel reluctant or uneasy to be without your smartphone, even for a short time?

12. Do you feel uncomfortable, panicked, or ill at ease when you accidentally leave your phone behind or have no service?

13. When you eat meals, is your smartphone almost always part of the table place setting?

14. When your smartphone rings, beeps, or buzzes, do you feel an intense, immediate urge to check it?

15. Do you find yourself mindlessly checking your smartphone many times a day, even when you know there is likely nothing new or important to see?
